A "Triple Nipple" is the appearance, within one show, of the three Phish songs that mention nipples (and nipple slicing, by the way):
Additionally, "The Oh Kee Pa Ceremony" was named for a tribal ceremony that's painful to nipples. And at least one version of "Sanity" (on Junta) includes the line, "You can slice my nipple.". Some fans would count either or both of those.
Only two shows have included the canonic three above (8/17/89 and 10/26/89). Being looser about the definition permits perhaps 34 "Triple Nipples", including for example:
